1. Animal Care Trust USA will place your loved pet with an approved Forever Family or Forever Sanctuary partner.
2. Up to $100 per year for covered medical expenses for small mammals, $250 per year for cats, $350 per year for dogs, $500 per year for horses.
Covered medical expenses include veterinary visits for routine, chronic, or emergency care, including annual veterinary certificate, monthly flea and tick prevention, heartworm prevention, and prescriptions.
3. Annual Healthy Pet Vet Check – ACT USA will require your loved pet’s Forever Family or Forever Sanctuary to provide annual veterinary evidence your loved pet is receiving proper love and care. (Veterinary certification required)
4. Forever Loved Journey’s End – At the end of your loved pet’s lifetime, this includes clinic or at-home euthanasia through an approved partner or service provider with cremation for small mammals, cats, and dogs, and final disposition of remains. It includes on-site burial or cremation and final disposition of remains for horses, at the discretion of ACT USA.
1. Small mammals – $5,000
2. Cats – $10,000
3. Dogs – $15,000
4. Horses – $50,000
If you have exotic pets, birds, or reptiles, call us for minimum recommended funding requirements.
With the Forever Loved Pet Trust, you can:
1. Download the Forever Loved Pet Trust and then select your own pet caregivers, pet trustee and remainder beneficiary (enrollment in the Forever Loved Pet Protection Plan is not required; OR
2. Animal Care Trust USA will place your pet with a Forever Family or with one of our Forever Sanctuary partners. We will also act as pet trustee. (Forever Loved Pet Protection Plan required)

2. Up to $200 per year for covered medical expenses for small mammals, $500 per year for cats, $700 per year for dogs, $1,000 per year for horses.
Covered medical expenses include veterinary visits for routine, chronic, or emergency care, including annual veterinary certificate, monthly flea and tick prevention, heartworm prevention, and prescriptions.
3. Monthly Healthy Pet Check with Annual Healthy Pet Vet Check – ACT USA will “visit” with your pet monthly via electronic media and will require your loved pet’s Forever Family or Forever Sanctuary to provide annual veterinary evidence your loved pet is receiving proper love and care. (Veterinary certification required)
4. Forever Loved Journey’s End – At the end of your loved pet’s lifetime, this includes clinic or at-home euthanasia through an approved partner or service provider with cremation for small mammals, cats, and dogs and final disposition of remains. It includes on-site burial or cremation and final disposition of remains for horses, at the discretion of ACT USA.
To name Animal Care Trust USA as your pet trustee, proof of minimum funding and enrollment in the Forever Loved Pet Protection Plan is required.

Step 2: Submit your pet trust for a Forever Loved Pet Trust Review ($500).
Step 3: Upon acceptance, enrollment in the Forever Loved Pet Protection Plan ($10 per month) is required.
